| mechanism | A DEFINITION DELIBERATELY LOOSENED, THREE TIMES, BY THE SAME PANEL: `These revisions simplify the Criteria, preserve their diagnostic sensitivity and specificity, address their applicability across populations, and may allow earlier diagnosis and more uniform and widespread use.` The corpus holds definitions tightened, abolished and kept for continuity. This one has been relaxed on purpose in 2001, 2005 and 2010, each time with the sensitivity/specificity claim attached— “New evidence and consensus has led to further revision of the McDonald Criteria for diagnosis of multiple sclerosis. The use of imaging for demonstration of dissemination of central nervous system lesions in space and time has been simplified, and in some circumstances dissemination in space and time can be established by a single scan. | mechanism | AND DISSEMINATION IN TIME IS NOW DEMONSTRABLE AT A SINGLE MOMENT: `in some circumstances dissemination in space and time can be established by a single scan` - because `a single brain MRI study that demonstrates DIS and both asymptomatic gadolinium-enhancing and non-enhancing lesions is highly specific for predicting early development of clinically definite MS`. A criterion whose name is TIME, satisfied without the passage of any - the two lesion types imply different ages, so one image carries a history— “By using the recommended simplified MAGNIMS criteria to demonstrate DIS 22 and allowing DIT to be demonstrated by a scan containing both enhancing and nonenhancing lesions in regions of the CNS typical for MS, 23 a diagnosis of MS can be made in some CIS patients based on a single MRI. The Panel felt this is justified because it simplifies… | mechanisms[1] |
| mechanism | AND A WAITING REQUIREMENT WAS DROPPED BECAUSE IT COST A SCAN: `The 2005 revision ... based [DIT] on the appearance of a new T2 lesion on a scan compared to a reference or baseline scan performed AT LEAST 30 days after the onset ... In clinical practice, however, there is reason NOT TO POSTPONE A FIRST MRI until after 30 days, which would result in an extra MRI scan to confirm a diagnosis. ABANDONING THE REQUIREMENT ... does not compromise specificity.` A criterion removed on cost, with the specificity claim cited beside it— “The 2005 revision of the McDonald Criteria simplified the MRI evidence required for DIT, basing it on the appearance of a new T2 lesion on a scan compared to a reference or baseline scan performed at least 30 days after the onset of the initial clinical event. In clinical practice, however, there is reason not to postpone a first MRI until… | mechanisms[2] |
| mechanism | AND A FURTHER LIBERALISATION WAS REFUSED, FOR A REASON ABOUT THE EVIDENCE BASE: `In the 2001 and 2005 McDonald Criteria, a positive CSF finding could be used to reduce the MRI requirements ... However, when applying the simplified MAGNIMS imaging criteria, the Panel believes that even further liberalizing MRI requirements in CSF-positive patients is not appropriate, as CSF status was not evaluated for its contribution to the MAGNIMS criteria.` A relaxation declined because the variable was not in the study the new rule came from - and an existing allowance withdrawn by the same reasoning— “In the 2001 and 2005 McDonald Criteria, a positive CSF finding could be used to reduce the MRI requirements for reaching DIS criteria (requiring only 2 or more MRI-detected lesions consistent with MS if the CSF was positive). 8 , 9 However, when applying the simplified MAGNIMS imaging criteria for DIS and DIT, 24 the Panel… | mechanisms[3] |

When we have changed our mind. Some entries in the table begin with a word in capitals and the reason for it. EVIDENCE CORRECTED — the finding still stands, but we had quoted the wrong sentence from the paper, and the quotation has been replaced with the right one. RETRACTED — the entry itself was wrong; it stays on the page, marked, with the reason, rather than being deleted. WITHDRAWN AND REPLACED — the entry was wrong, and a corrected version of it appears in the entries below. Each mark carries its own reason, in brackets, because the reasons differ from one entry to the next. An entry we have WITHDRAWN is removed from this page and kept in the record, with its reason. A line reading Audit and a date is a later re-reading of the entry against its whole sentence: DOUBTFUL means it was kept, and the reason says what is loose about it. Quotation marks inside the tagger's sentence. Where a tag is the tagger's own sentence, a phrase in quotation marks inside it is borrowed from the paper — the tagger's wording around it is not. The paper's own sentence is the evidence quote for that row; on this page it is withheld with the article, and the article is at its publisher. Nothing inside quotation marks anywhere on this page is ours: if it is quoted, it is the paper's. Whose words a concept carries. A concept entry records a term the paper itself uses, so the term, the sense given for it, and its evidence are the paper's own words, quoted where the paper fixes them. This is the opposite of a sense entry in the tag table, which is the tagger's sentence about how the paper uses a term. Where a concept entry could not be grounded in the paper's words, the entry says so on its own row.
